Business Membership Package
Complete these forms or visit the nearest branch to open a membership or new account.
Business Loan & Credit Card Applications
When you apply for a business loan or business credit card, we will ask for the last two years worth of tax returns for the business and your personal tax returns.
In addition to the Business Membership Package, each of the following types of businesses require additional documentation.
Sole Proprietorship/DBA
- Completed and signed Business Membership Package
- Employer Identification Number (EIN) confirmation letter from the IRS
- Business License, if available
- Certificate of Fictitious/Assumed name, if applicable
Partnership
- Completed and signed Business Membership Package
- Certification of Beneficial Ownership form
- Partnership agreement
- Employer Identification Number (EIN) from the IRS
- Business License, if available
- Copy of the Certificate of Partnership, if applicable
- Copy of the SCC registration, if registered with the SCC
- Certificate of Fictitious/Assumed name, if applicable
Unincorporated Organization
- Completed and signed Business Membership Package
- Certification of Beneficial Ownership form
- Articles of Organization
- Minutes of last meeting signed by the Association/Organization’s officer(s)
- Employer Identification Number (EIN) from the IRS
- Authorization to act on behalf of Association/Organization
Corporation
- Completed and signed Business Membership Package
- Certification of Beneficial Ownership form
- Articles of Incorporation
- Copy of the SCC registration
- Employer Identification Number (EIN) from the IRS
- Bylaws, if available

Limited Liability Company (LLC)
